Add Your Pet's Name to This Year's Program - No Matter Where You Live!

If you will be attending Honoring the Animals and would like to add your pet's name to our official program, you may do so for $10 (to help cover production costs). We will reserve a program for you to be picked up at the event. IF YOU CANNOT ATTEND you may still add your pet's name and a program will be sent to you via US Postal Mail. Note: HTA Programs will be mailed on or about Monday, September 13, 2010.

2. If you wish to submit more than one pet name, please submit one at a time. A $10 donation is required for EACH PET NAME ADDED. We will contact you regarding whether you would like them placed together or on separate lines.

3. BE SURE TO CHECK ALL INFORMATION AND SPELLING FOR ACCURACY. CHANCE’S SPOT IS NOT RESPONSIBLE FOR ERRORS OR MISSPELLINGS.

4. Submit the form to confirm on the next page. Once you have confirmed your information you will be asked to make your payment using a credit card, eCHECK or PayPal to Chance's Spot.*
